CIGNA Medical HomeQuality Incentive Plans | Standard Plans |

The following ACSUM Health Plan changes will take effect on November 1, 2012 as recommended by the Employee Health Plan Task Force and included in your recently negotiated and ratified collective bargaining contract.  Unit members are encouraged to attend an on-campus meeting to review these improvements.

Health Plan Changes

Effective November 1, 2012, ACSUM unit members will be automatically transitioned to the Plan that is most similar to their current plan and will receive a new ID card from CIGNA.

Current Plan      ->     Plan effective November 1, 2012

Standard “Copay” Plan   -> Quality Incentive “Copay” Plan (SPD)

Standard “Passive” Plan -> Quality Incentive “Passive” Plan (SPD)

You will be given the chance to change plans during the Fall 2012 Open Enrollment period (scheduled for November 1 through November 25) and those elections will be effective January 1, 2013.

Choosing a "quality" Primary Care Physician (PCP) and hospital will ensure less out-of-pocket expenses and better outcomes for those enrolled in the Quality Incentive health plans.  Visit the Quality Incentive Health Plan webpage for additional information about these plans.

Financial Incentives for Participation in the RiseUP, The UMS Wellness Program

In order to "incentivize" the UMS employee population to get engaged in personal health improvement, the following premium schedule was adopted.

  • If you DO “participate” in RiseUP, you will pay 10%* of the total health plan premium

  • If you do NOT “participate” in RiseUP, you will pay 20%* of the total health plan premium

*Average Full-time regular employee premium share - see ACSUM Rate Chart for part-time employee rates

Normally, Phase I participation takes place between January 1 and March 31 of every year.  To complete Phase 1, login to your Reasons2RiseUp account at (https://www.ums2riseup.com/).  For more information about Phase 1,

watch mail, email and visit http://www.umsriseup.maine.edu for details or call APS Healthcare at 855-2RISEUP (855-274-7387).

ACSUM unit members have 90 days from November 1, 2012 to engage in the program and earn the incentive rates.  If you already completed Phase I of RiseUP January 1, 2012 – October 31, 2012, you do not have to do anything further and the premium incentive will automatically begin with your November 2012 paycheck. See RiseUP Incentive Schedule and Health Premium Rate Chart for additional information and timelines.

From November 10, 2012 – January 31, 2013

  • If you DO NOT complete Level 1 of RiseUP, you will pay approximately 12% during the 90 day period, and then pay the higher 20% premium as of February 10, 2013.

  • If you DO complete Level 1 of RiseUP, you will qualify for the lower 10% premium as of the following month. This 10% rate will continue until March 31, 2013.

  • If your spouse/partner is enrolled in the health plan, they must also complete Level 1 for you to qualify for the incentive.

  • If you complete Level 1 and then complete Level 2 of RiseUP (by accumulating a total of 120 points) by December 15, 2012, you will receive $100 ($200 if your spouse or domestic partner is enrolled and also completes Level 2). Please review the enclosed RiseUP Program Incentive Schedule for more details.

From January 1, 2013 – March 31, 2013

  • We will contact you to encourage you to participate in the RiseUP program for 2013.

  • If you DO NOT complete Level 1 by March 31, 2013 your premium will increase to 20% effective April 10, 2013.

  • If you DO complete Level 1 by March 31, 2013, you will pay the reduced 10% premium effective April 10, 2013.

  • If your spouse/partner is enrolled in the health plan, they must also complete Level 1 for you to qualify for the incentive.

  • Once you complete Level 1, you can move on to complete Level 2 during 2013 and qualify for another $100 payment ($200 if your spouse or domestic partner is enrolled in the health plan and completes Level 2). Level 2 payments will be processed later in 2013.
